Description General Job Summary The Clinical Pharmacist assumes responsibility for the medication management of assigned patients; monitors and evaluates medication therapy for appropriateness based on the patient's health status, institutional criteria /guidelines, and cost efficient measures including medication reconciliation, medication monitoring and patient education; remains integrally involved in the education of Ph! armacy and Medical Residents and provides education to health ! care providers as needed; and provides pharmacy expert content for order sets, benefit design, formularies, and other pharmacy related documents as needed. Required Qualifications Education / Certifications / Licensure / Specialized Training Graduate of an accredited School/College of Pharmacy
PGY1 Residency in Pharmacy Practice or PGY2 Residency or equivalent experience
Licensed to practice pharmacy in the State of Louisiana or eligible for licensure by examination or reciprocity form another state Experience / Skills Interpersonal skills to deal effectively with all members of the health care team
Skills to operate a computer including word processing, presentation, and spreadsheet programs
Preferred Qualifications
Experience / Skills / Competencies Leadership and teaching skills Essential Position Duties 1. Provides Patient Assessment and Care
2. Conducts Pharmacoeconomics
3. Conducts Education and Scholarship Activities
