Assists in establishing and coordinating all clinical pharmaceutical services for Tulane Hospital and to include: development and continuous revision/update of clinical policies and procedures a! nd JCAHO clinical standards preparedness; development and monitoring (and documentation) of cognitive pharmacy services including therapeutic interchange and formulary management, antibiotic utilization, Medication Use Evaluation, Adverse Drug Event (ADE/ADR) monitoring, Occurrence Report monitoring specific to clinical practice and continuous improvement of clinical pharmacy services and clinical skills of distributive staff; consultation with medical staff and patient care providers as an advisor/educator regarding clinical pharmacy programs and optimal pharmaceutical care. Participates in strategic planning and goal setting for the Department of Pharmaceutical Services. Performs all other duties as assigned
A. Licensure/Certification/Registration: Louisiana licensed pharmacist or eligible.
B. Education: Bachelor of Science degree in Pharmacy from a college accredited by the American Council on Pharmaceutical Education (ACPE) required. Advanced ! degree (PharmD, PhD) , ASHP/ACCP accredited Residency or equiv! alent preferred.
C. Experience: Minimum of 2 year's post-graduate clinical pharmacist experience in institutional pharmacy; preferably 5 or more years of progressive clinical experience.
D. Special qualifications: Must be able to successfully complete established competencies for the position within designated probationary period. Board Certified in a pharmacy practice specialty preferred. Demonstrated excellent verbal and written communication skills. Word processing, spreadsheet, and other computer software skills desirable.
