The Pharmacist shall provide services to patients while maintaining a high level of clinical competence in their specialty practice. Services are to be provided to the Bayne Jones Army Community Hospital and surrounding clinics supported at Ft. Polk, LA.

Essential Duties and Responsibilities

Check prescriptions for patient eligibility, drug availability, usage, dosage, mode and ! rate of administration, completeness of directions and possibl! e incompatibilities noted by reviewing the patient’s medication profile.
·
Fill prescriptions, including controlled drug prescriptions by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed, compounding when necessary, and packaging the drug in an appropriate container.
·
Exercise care in dispensing of drugs, especially controlled substances to assure that legal and agency regulations and instructions are followed, that proper and complete record keeping is maintained, and that the drug storage areas are stocked safe and secure.
·
Notify prescriber, who may be on staff or a community physician, when there are questionable dosages, or possible therapeutic incompatibilities based on a review of the patient’s medication profile.
·
Consult with and advise nurses and members of medical staff of matters such as contraindications, side effects, and antagon! istic reactions, and suggest appropriate alternative medications.
·
Determine validity of drugs prescribed by the numerous physician extenders, ensuring the drug prescribed is on the particular physician extender’s list.
·
Provide guidance to professional government personnel on current Pharmacy and Therapeutics Committee policies applicable to Federal and State laws, Army formulas, new prescriptions, drugs of choice, methods of compounding non-standard preparations and special formulas suggested or requested by the professional medical staff.
·
Dispense prescriptions to patients, counseling patients concerning medications, i.e., uses, side effects, and directions.
·
Routinely refer to standard reference texts and journals to keep abreast of current trends, new drugs, and new uses for current drug.
·
Verify information from prescriber when drug strength is questionable.
! ·
Interpret and evaluate unit dose orders, detect potential ! therapeutic incompatibilities and errors, insure unit dose orders are picked up from nursing units, review and approve technician’s work.
·
Fill and deliver unit dose cassettes, enter orders into computerized or manual patient profiles, place medication into unit dose cassettes, and deliver them to nursing unit.
·
Performs medication regimen review of all current and new members of the Warrior Transition Unit (WTU).
Reviews medication regimen whenever a new medication is added to a member of the WTU.
·
Interpret sterile product orders, detect therapeutic problems, employ sterile technique in preparation, accurately prepare product and monitor for particulate material.
·
Assist and perform the duties required by the Soldier Readiness Processing (SRP) to prepare deploying Soldiers requiring medications and the coordination needed for those Soldiers to receive those medications.
! This includes:
Briefing the Soldiers on proper procedures, entering medications into the Soldiers medication record, and preparing the forms needed to enroll those Soldiers requiring medications in The Mail Order Pharmacy (TMOP).

Compound medications for inpatient or outpatient use that require extemporaneous manufacturing procedures.
·
Provide technical guidance to pharmacy technicians.
·
Conduct pharmacy liaison visits assisted.
·
Insure pharmacy supplies are properly maintained and available to accomplish tasks.
·
Follow established guidelines/policies for authenticating records and maintaining prescription files to include controlled drugs and investigational drugs.
·
Assists with in-service training of government professional and technical pharmacy personnel and act as preceptor for pharmacy technicians.
·
Participate in Quality! Improvement (QI) activities as required by the MTF and Department of P! harmacy to meet health care goals and comply with guidelines established by the Joint Commission on Accreditation of Healthcare Organizations.

Prepare all records and reports, as required by MTF.

Be a graduate of an accredited College of Pharmacy.
·
Be licensed as a pharmacist with a current, valid, full and unrestricted license in one of 50 states and/or the District of Columbia, the Commonwealth of Puerto Rico, or territories of the U.S. and be able to obtain privileges at BJACH through approval of the credentials committee.
·
Have a minimum of one year of licensure expe! rience within the last three years.
·
Disqualifiers:
Chemically dependent or impaired, previously convicted of a felony, or waiting possible adverse administrative action by any Board of Pharmacy.
·
Have and maintain current certification in cardiopulmonary resuscitation (CPR) or basic cardiac life support (BCLS).
This must be obtained through the American Heart Association.
·
Fully comprehend and be fluent in speaking and writing the English language.
·
Possess proof of current (within 1 year) competence in the form of reference letters (3) from pharmacists.
·
Possess evidence of continuing pharmacy education which maintains skills and knowledge in the pharmacy specialty.
·
Knowledge of the full range of professional principles and practices of pharmacy.
·
Skill in counseling and teaching patients and significant family me! mbers regarding drugs.
·
Knowledge of authorized pha! rmaceuticals, their desired effects, side effects, and complications of their use.
·
Knowledge of the drugs and their possible reactions in order to clearly advise the patient accordingly of possible adverse drug side affects requiring medical intervention.
·
Must be able to be fully credentialed and licensed and be able to show proof of these along with references
·
Ability to reason and make quantifiable quality decisions based on pharmaceutical knowledge
Knowledge of current regulatory, licensing and legislative practices in pharmacy

Knowledge, Skills and Abilities:

Knowledge and comfortability with computer software and databases.
·
Able to learn CHLTA, and other government database systems where applicable.
·
Knowledge of Joint Commission, HIPAA and Privacy Act confidentiality statutes and regulations.
