DUTIES:
Provides consultation to physicians, dentists, nurses, and
other health care professionals on basic information about drug therapy.

Evaluate medication orders for
appropriateness of therapy; incompatibilities, side effects and interactions of
drugs and drug products, and resolves drug-related questions with the
prescriber.

Provides patient
consultation by reviewing patient profiles and medication orders.

Evaluate patientÂ's ability to understand
medication instruction.

Maintains record
of drugs, allergies, and other pertine! nt information in patientÂ's medication
profiles.

Supervises Narcotic Pharmacy
Technician. Dispenses all medication and supplies to outpatients in accordance
with local and VA policy, professional standards, and the latest available drug
information.

Maintains complete records
of drugs, allergies and other pertinent information in patientÂ's medication
profiles, and ensure accountability of prescribed drugs and supplies.

Dispenses, in a timely manner, narcotic
controlled substances to patients and to the hospital nursing units and
clinics.

Insures, when mail out is
requested, that all Schedule II and III narcotic substances are sent by
certified mail only.

Within 7 days of
expiration or procedural change, prepares and updates for the Pharmacy

ManagerÂ's signature, all written procedures
covering controlled substances.

Handles problems
referred by the ancillary ! employees in pharmacy, and follows problems through
to the conclusi! on or resolution.

Assists
in gathering data for statistical and managerial reports.

Assumes complete responsibility for the Pyxis
automated dispensing equipment.

Assures
timely refilling and reconciliation of controlled substances in the Pyxis
machine.

Trouble shoots problems with
automated dispensing equipment and edit users.

Responsible for
timely and accurate production and maintenance of all records required by VA,
DEA, and pharmacy procedures in the procurement, storage, inventory and disposition
of controlled substances.

Insures that
inventories are routinely reviewed by assess needs, remove excess,
deteriorated, or out-of-date drugs.

Destroys expired controlled substances according to federal
guidelines.

Orders and maintains stock
for outpatient and selected inpatient narcotics.

Assumes responsibility for control
prescription pads throu! ghout the facility.

Acts as clinical supervisor for professional or technical level pharmacy
students if required.

Maintains
professional competence through active participation in continuing
education.

Provides in-service program
in areas of expertise.

Orders,
dispenses, and keeps medication records for investigational studies.

QUALIFICATIONS REQUIRED:
VA Handbook 5005/36, Part II, Appendix G15.

BASIC REQUIREMENTS. Requirements for appointment as a Licensed Pharmacist in VHA are as follows:

Education

(1)

Graduate of a degree program from an approved college or university. The degree program mu! st have been approved by the American Council on Pharmaceutical Educati! on (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP).

(2) Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following:

(a) A letter of acceptance into a United States graduate pharmacy program recognized by the ACPE.

(b) Written certification from the Foreign Pharmacy Graduate Examination Commission, 700 Busse Highway, Park Ridge, IL 60068, Phone (947) 698-6227, that the individual has successfully passed the Foreign Pharmacy Graduate Examination.

(c) A letter from a United States college or university with a pharmacy degree program recognized by ACPE stating that the individual's pharmacy degree has been evaluated and found to be equivale! nt to its Bachelor of Pharmacy degree or higher.

Licensure.

Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the

United States

(e.g., Puerto Rico), or the

District of Columbia

.

Grade Determination - GS-12:
Experience.

Completion of the equivalent of one year of creditable experience at the next lower grade level which is directly related to the position to be filled or completion of a one year post-Pharm.D. ASHP accredited Residency.

In addition to meeting the basic requirements, pharmacists at this grade level must demonstrate the following KSAs:

1. Comprehensive knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy such as quality assurance/utilization review, informatics systems, drug information, etc. This level of knowledge is usually characterized by at least on! e year of experience in a specialized clinical area or advanced trainin! g/certification;

2. Comprehensive knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, pharmacoeconomics and pharmacotherapeutics. This level of knowledge is usually characterized by by at least 1 year of experiencein a specialized clinical area or advanced training/certification;

3. Skill in monitoring and assessing the outcome of drug therapies including physical assessment and interpretation of laboratory and other diagnostic parameters;

4. Knowledge of the design, conduct, and intrepretation of controlled clinical drug trials or other research related to health care;

5. Ability to communicate orally to persuade and influence clinical decisions;

6. Ability to communicate in writing to persuade and influence clinical decision;

7. Comprehensive knowledge of DEA, FDA, VA,

the State (Territory of District of Columbia, if appropriate), TJC and other standards related to the distribution and control of s! cheduled and non-scheduled drugs to both inpatients and outpatients (including research and investigational drugs).

ENGLISH LANGUAGE PROFICIENCY:
Pharamcists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).

PROBATIONARY PERIOD:
This position requires completion of 1-yr probationary period, if not already served.

DRUG-FREE WORKPLACE:
The VA has established a Drug-Free Federal Workplace Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment. Applicants who refuse to be tested will be denied employment with the VA for a period of six months.
