Provides quality patient care in relation to the patient’s oncologic diagnosis, prescribed treatment, age group, and other identified needs. Provides comprehensi! ve pharmaceutical care to oncology patients to assure safe and effective drug therapy. Requires independent decision-making, clinical judgment, and in-depth knowledge of oncology and therapeutics.
Scope
Coordinates the drug therapy process on oncology patient population. May include any of all of the followingactivities: drug selection, drug information, dosing, monitoring, outcomes management, and patienteducation/counseling.
Essential Job Functions
1. Develops and maintains a clinical practice with a patient care service, cooperating with medical and nursing personnel to optimize the pharmacotherapeutic aspects of patient care. Provides optimal selection
of pharmacologic agents, and design of a regimen to maximize therapeutics benefit and minimize toxicity. Provides daily follow-up on the clinical effects of the regimen with adjustments as necessary to achieved desired outcomes.
2. Provides drug and clinical informatio! n to other healthcare professionals in response to specific qu! estions relating to new evolving issues related to the care of patients with cancer. Provides professional education regarding new drugs, biologics, or technologies for specialized drug delivery. Provides a significant level of education to professionals, trainees, and patients. Responsible for monitoring and assessing performance of the learner and providing feedback using appropriate learning objectives that are aligned with the desired learning outcomes.
3. Performs pharmacokinetics dosing and monitoring for all patients on drugs where therapeutics levels are reported.
Education
Required: PharmD. Degree
Experience
Required: PGY1 and PGY2 Residency or the equivalent experience as Clinical Pharmacist in the area of Oncology.
