The Alexandria VA Health Care System is seeking a Clinical Pharmacist at the Community Based Outpatient Clinic in Jennings, LA. The Alexandria VA Health Care System, Alexandria, Louisiana is a Joint Commission accredited, complexity level 3 facility serving Veterans within 23 of LouisianaÂ's 64 parishes. The VAMC is located in Pineville, Louisiana with community based outpatient clinics (CBOCs) in Jennings, Lafayette, Fort Polk, and Natchitoches, Louisiana.

DUTIES:
This position functions as a staff pharmacist with clinical duties providing pharmaceutical care to inpatient areas of the Alexandria , La. VAMC. Performs all duties assigned to a staff pharmacist relating to verification of prescriptions, filling, and medication counseling when necessary. ReviewÂ's the patientÂ's drug regimen for appropriate indication, dosing, allergy verification, interaction potential, lab monitoring and non-VA medication documentation. Works as a mid level provider with duties assigned in program areas of anti-coagulation, lipid, and hypertension management. Participates as a member of the Primary Care Team, documenting interaction through the use of progress notes in CPRS. Manages drug and supply expenditures in an efficient, effective, and accountable manner. Provides a clinical review of non-formulary requests. Approval or Denial must be clinically based decisions that comply with national PBM criteria for use as interpreted by the local facility Pharmacy Chief. Must also provide follow up review of effectiveness according to guidelines to allow continued dispensing of non-formulary medication. Provides complete patient education to patients and family members when appropriate. Provides consultation upon request in areas such as drug therapy selections, pharmacokinetics, parenteral nutrition support, and determination of therapeutic endpoints. Actively participates in the education of healthcare professionals. Assists in the management and documentation of adverse drug reactions and medication errors.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
A. U.S. Citizenship: Must be a United States Citizen

B. Education:
(1) Graduate of a degree program in pharmacy from an approved college or university.

The degree program must have been approved by the American Council on Pharmaceutical Education (ACPE), or prior to the establishment of ACPE, have been a member of the American Association of Colleges of Pharmacy (AACP). Verification of approved degree programs may be obtained from the American Council on Pharmaceutical Education, 311 West Superior Street, Suite 512, Chicago, Illinois 60610-3537 Phone: (312) 664-3575), or through their Web site at: http://www.acpe-accredit.org/ .

(2) Graduates of foreign pharmacy degree programs or those who graduated from a U.S.-based non-ACPE accredited degree program meet the educational requirement if their degree is found to be equivalent to degree programs recognized by the ACPE. This finding may be based on any of the following:

(a) A letter of acceptance into a United States graduate pharmacy program recognized by the ACPE.

(b) Written certification from the Foreign Pharmacy Graduate Examination Commission, 700 Busse Highway, Park Ridge, IL 60068, Phone (847) 698-6227, that the individual has successfully passed the Foreign Pharmacy Graduate Examination.

C. License : Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

D. English Language Proficiency: Proficiency in spoken and written English language is required for direct patient care positions by 38 U.S.C. 7402(d) (P.L.95-201)

GRADE DETERMINATIONS:
For the GS-12 level, applicants must have the following specialized experience:

Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled or completion of a 1 year post-Pharm.D. ASHP accredited Residency.

For the GS-13 level, applicants must have the following specialized experience:

Completion of the equivalent of 1 year of creditable experience at the next lower grade level which is directly related to the position to be filled, or board certification recognized by the American Pharmaceutical AssociationÂ's Board of Pharmaceutical Specialties, or the American Society of Consultant Pharmacists (or equivalent), or completion of a two year post Pharm.D. fellowship in a clinical or specialized program.

PHYSICAL REQUIREMENTS: The work involves prolonged periods of standing with some walking of generally short distances. There may be occasional lifting of weights of up to 30 pounds. There may be mental fatigue due to repetitive screening and interpretation of medication orders.
